A Louisiana-based Medicare Advantage and D-SNP plan faced the challenge of improving health outcomes for its high-risk members managing chronic conditions. To address this, the plan partnered with Wellth, a behavior change engine that uses behavioral science and digital innovation to motivate members.
The solution from Wellth involved members using an app to submit daily photo check-ins of their health tasks, like glucometer readings, in exchange for rewards for essential items. This program drove significant results, including a 10% improvement in medication adherence for some conditions and a 38% reduction in readmission days compared to non-enrolled members. Wellth also demonstrated a $96 PMPM inpatient savings and delivered over $435,000 in rewards directly to members to address social needs, achieving a +79 Net Promoter Score.
