EPIC Management, a provider group shouldering full risk for its Medicare Advantage populations, faced a challenge in engaging patients, particularly those in remote communities, in care plan adherence programs for chronic conditions like diabetes and CHF. Wellth implemented a solution where patients used its app for daily medication check-ins, submitting photos verified by AI. Members were motivated by a financial incentive that leveraged loss aversion, penalizing missed days. This approach achieved a 94% average daily adherence rate. The program resulted in significant measurable impacts: a 38% reduction in ED utilization, a 22% reduction in inpatient utilization, a 68% reduction in bed days, and, for diabetic patients, an average HbA1c reduction of 0.61.
