Partnering with community organizations to address social determinants of health

Flagler Health+ Care Connect partnered with WellSky to address social determinants of health in St. John’s County, Florida. Faced with complex, fragmented community services, high inappropriate emergency department use, and the need to better coordinate care across providers, the organization sought a more efficient way to connect residents to housing, food, medical, behavioral health, transportation, and other support services.

Using WellSky Community Services, Flagler Health+ Care Connect implemented a single intake, assessment, eligibility screening, referral, and care coordination system across 40 participating organizations. With WellSky, Care Connect cut time to access services from up to 10 days to less than 48 hours, helped providers address service needs five times faster, reduced the onboarding process for new organizations from six months to six days, and improved outcomes including decreases in unsheltered homelessness, veteran homelessness, youth homelessness, hospital readmissions, and time to behavioral health appointments.
