Concordia Plans, which supports workers of The Lutheran Church—Missouri Synod, wanted to improve member health, reduce chronic risk, and boost sustained engagement across its diverse ministries. To address this, Concordia Plans partnered with Vitality, leveraging Vitality’s core offerings—especially its physical activity program—and the platform’s flexible ecosystem to integrate tailored third-party services and guide members to the right supports.
Vitality deployed a point-based rewards model, triage-driven referrals, integrated external programs, and targeted communications to keep members active and progressing through status levels; Concordia members reaching Gold/Platinum reported better health and workplace outcomes. The Vitality partnership produced measurable results: a 7.5% better-than-expected improvement in risk groupings and status-linked impacts (59% lower Vitality Age, 46% fewer health-related absences, 19% better perceived health, 10% lower turnover, and 4% higher job satisfaction).
