Using MCAP to Understand Delays in Medication & Pharmacy Liverpool Heart and Chest Hospital NHS Foundation Trust

Liverpool Heart and Chest Hospital NHS Foundation Trust worked with VitalHub to address delays in patient discharge caused by medications not being dispensed in time. Using VitalHub’s MCAP data, the trust identified that patients were often leaving later in the day because pharmacy processes were slowing discharge.

VitalHub implemented a pilot on a 42-bedded cardiology ward, assigning a pharmacy ward technician each morning to reduce medication delays and improve coordination with pharmacy and ward teams. The pilot led to fewer phone calls to pharmacy, faster handling of urgent TTOs, more patients discharged before lunch, quicker resolution of medication issues, and reduced medication wastage, with estimated annual savings of just over £40k; medically optimised patients needing TTOs also fell from 46% to 29% in one month.
