Using MCAP for Discharge Planning The Walton Centre NHS Foundation Trust

The Walton Centre NHS Foundation Trust worked with VitalHub using MCAP to identify and address delays in medication and pharmacy processes, particularly around TTOs (to-take-out medicines). The case study showed that the Trust’s discharge delays were being caused by internal factors, including no weekend cover for dispensing TTOs and ward round timings that often pushed back orders and pharmacy collections.

VitalHub implemented several changes, including sharing a list of prescribers with all wards, adding an extra pharmacy collection round at 12 pm, and exploring pre-packed drug options. As a result, TTOs were no longer a top five internal delay for the Trust, and overall internal delay days reduced by 48%.
