SIU Medicine, a multispecialty academic physician practice in Springfield, Illinois, needed better standardization and reporting for preventative care documentation as it transitioned to an EHR. With Veradigm, SIU Medicine created a cross-functional workflow, standardized flowsheets, and piloted the process across primary care sites using Lean Six Sigma methods. The results included fall risk screening rising from 17% in 2016 to nearly 78% in 2017, depression screening increasing from 3% to 49%, and pneumococcal vaccination status in older adults improving from 65% to 80%, with several measures surpassing national benchmarks.
