Delta Health Alliance, a nonprofit collaboration of healthcare providers in Mississippi, faced the challenge of increasing access to care and improving outcomes for a large diabetic population in a medically underserved region. To address this, they partnered with Veradigm, utilizing a suite of solutions including the TouchWorks EHR, Care Director, and the FollowMyHealth patient engagement portal.

By implementing Veradigm's customizable EHR and care management tools, the Alliance developed specialized "Care Guides" that standardized care. This solution drove significant results: it helped show a statistically significant decrease in blood sugar levels, reduced hospital readmissions to 3.1% (versus a 26.3% national average), and generated an estimated $300,000 in savings. Furthermore, all providers achieved Meaningful Use attestation, earning $2 million in incentive funding.
