They utilized the Veradigm FollowMyHealth patient portal as a key solution.

Veradigm implemented the FollowMyHealth platform, customizing it with fields for patients' preferred names, pronouns, and Sexual Orientation and Gender Identity (SOGI) data. This sensitive approach fostered a stronger, more comfortable relationship between providers and patients. The solution achieved a 75% patient adoption rate and enabled a rapid shift to telehealth, with 90% of visits becoming virtual during the COVID-19 pandemic. The integration also ensured consistent data across all systems, allowing for more accurate clinical monitoring and follow-up for over 1,200 patients.
