Connecting physician groups for better patient care at Holston Medical Group

Holston Medical Group (HMG), a large multi-specialty physician group, faced the challenge of improving patient care and succeeding in value-based contracts. This required connecting disparate physician groups and normalizing data from various EHR systems to gain a complete view of patient histories. To address this, HMG collaborated with Veradigm, leveraging solutions like the dbMotion health information exchange platform.

The solution implemented by Veradigm, through the OnePartner HIE powered by dbMotion, enabled secure data sharing across 13 different EHR systems connecting over 1,500 physicians. This gave providers a unified view of patient information, which improved care coordination and enabled more accurate coding. The results were significant; HMG earned $7.2 million in value-based payments in 2015, and its ACO, Qualuable, generated $40 million in shared savings. Participants using the Veradigm-enabled HIE also saw a 20% increase in risk scores, a 5% increase in quality scores, and a 12% increase in net revenue.
