Veradigm's solution provided a comprehensive care team with access to patient data to conduct outreach, monthly medication reconciliation, and care coordination via telephone. This program enrolled over 400 patients in its first three months. The initiative is projected to generate $1.3 million in new revenue for the medical center over three years while aiming to reduce ER visits and hospital readmissions.
