A new vision for comprehensive care coordination

Holyoke Medical Center, a community hospital, faced the challenge of disconnected care management between its inpatient and outpatient departments. The results were significant, with Veradigm helping Holyoke Medical Center reduce its readmission rate, increase patient satisfaction from 88.5% to 92.3%, and achieve a substantial improvement in its Care Transition score from 54.4% to 60.4% within just six months.
