Yale-New Haven Hospital, a leading teaching and tertiary care center with more than 4,500 physicians and practitioners, faced a complex, multi-stakeholder patient transfer process that lacked a real-time, centralized way to manage, track, and communicate cases. To support its goal of becoming a regional destination hospital and to increase operational efficiency and referrals, Yale-New Haven Hospital engaged Velosio to deploy Microsoft Dynamics CRM Online as the core platform to address these challenges.

Velosio customized and integrated Dynamics CRM Online with multiple data repositories and the phone system, built dashboards and KPIs for real-time transfer visibility, and helped launch a 24/7 “Y Access Line” to connect referring physicians with live clinicians. As a result, Yale-New Haven completed 700 transfers in the first four months and now averages 250+ transfers per month, with faster clinician-to-clinician coordination and improved bed assignment and acceptance tracking.
