Delaware Valley Community Health Leverages RPM to Support Community-Wide Hypertension Initiative

Delaware Valley Community Health (DVCH), a network of nine Federally Qualified Health Centers serving uninsured and underinsured patients in Southeastern Pennsylvania, sought to control hypertension outside the clinic and reduce disparities in its African American population. To capture blood pressure data during telehealth and a HRSA-funded community outreach called Road to Controlled Hypertension, DVCH deployed a Self‑Measured Blood Pressure (SMBP) program and selected NextGen Remote Patient Monitoring powered by Validic Impact (Validic) to support enrollment and data capture.

DVCH used NextGen RPM with Validic’s real‑time display and Bluetooth cuffs for easy in‑app enrollment, EHR interoperability, and shared patient data visibility for care teams. With Validic-enabled RPM, DVCH outreached 1,193 patients by June 2022, enrolled 358 (244 graduated by Sept 2022), and exceeded its year‑end hypertension control goal—reaching 55.3% controlled versus the 48% target—while giving providers more actionable data and addressing social determinants of health through coordinated supports.
