Inova, a major nonprofit health system in Northern Virginia, faced the challenge of addressing patients' social drivers of health (SDOH) with inefficient, outdated methods like static resource lists and manual referrals. These processes were time-consuming and provided no visibility into whether patients actually received help, creating a gap between identifying needs and taking action. To solve this, Inova partnered with Unite Us to implement a technology-enabled care coordination strategy.

The solution involved deploying Unite Us's closed-loop electronic referral system, Unite Virginia, to streamline and track referrals to community-based organizations. This was supported by a strong focus on community partnership, network engagement, and data-driven insights. As a result, Inova screened over one million patients and facilitated thousands of successful referrals, achieving a 76% acceptance rate and a 67% resolution rate from community partners. Unite Us enabled Inova to build a more accountable system that improved whole-person health outcomes and strengthened community collaboration.
