Twistag built a multi-tenant clinical trial platform for Worthmed, a clinical research organisation. Worthmed's challenge was that the protocol design phase for clinical trials was a fragmented process managed through email and spreadsheets, consuming 12–16 weeks and delaying patient access to new treatments.

The solution implemented by Twistag was a unified platform featuring a normalized PostgreSQL schema and a Blazor WebAssembly interface on a .NET Core backend. This solution compressed the protocol design time from 12–16 weeks down to 6–8 weeks, provided FHIR-compliant interoperability with hospital systems, and turned regulatory submission into a documented process rather than a reconstruction.
