A Tribridge Case Study
Virtua, South Jersey’s largest health system, launched its VirtuaCare ACO to deliver more personalized, coordinated care for a growing Medicare population. To do that, it needed a new infrastructure and tools that could generate patient alerts, track enrollment, assign care managers, and support its accountable care goals. Tribridge provided the technology services and a customized Microsoft Dynamics CRM-based solution to help Virtua meet these needs.
Tribridge implemented its CareCoordination solution on Microsoft Dynamics CRM, integrating care alerts, registries, patient assignments, and administrative workflows into a unified platform. The result was a more proactive care model with better visibility into patient data, and VirtuaCare launched in January 2013 with 80 physicians and 14,000 Medicare patients, later expanding to 150,000+ patients. The program has shown favorable trends toward its clinical goals, including improved chronic care and greater patient engagement.
