Jefferson Health (formerly Kennedy Health), a three-hospital, 607‑bed system in southwest New Jersey, discovered extreme ED overutilization when 21 patients generated 903 emergency visits in six months. The organization’s challenge was to reduce repeat ED visits, unnecessary testing and medications, and avoidable readmissions across multiple campuses while coordinating care in real time.

By deploying TigerConnect and integrating it with the EHR, Jefferson implemented enterprise‑wide automated alerts and mobile communication so nurse navigators could intervene at check‑in and coordinate behavioral‑health or substance‑use referrals.
