A TigerConnect Case Study
Innovation Care Partners (ICP) is a physician‑led Clinical Integration Network and ACO in Arizona—serving more than 1,200 practice locations and six hospitals—that grew from about 1,500 covered lives in 2012 to over 100,000 and 2,000 physicians. ICP faced fragmented, inefficient communication across providers and needed a secure, easy‑to‑use way to coordinate chronic care and transitions to meet Medicare Shared Savings Program goals, cut readmissions, and lower overall cost of care.
ICP rolled out TigerConnect’s HIPAA‑compliant Clinical Collaboration Platform across clinical and nonclinical staff, adding EMR/LIS integration, group messaging, and voice/video to streamline discharge coordination and care transitions. The results: healthcare costs were cut roughly in half (2012–2014), readmissions fell by 50%, avoidable ED visits and SNF days declined, and ICP saved about 16.9% of MSSP spend (≈ $19.5M), ranking third of 472 ACOs for savings percentage. Additional outcomes included higher patient activation, reduced severe depression rates, and improved quality‑of‑life scores.
