Aegis Homecare and Hospice provides home-based post-acute and hospice care across Arizona, Utah, and Alaska with interdisciplinary teams of nurses, therapists, social workers and more. Coordinating care among many internal and external providers — while keeping patients and families from being overwhelmed — created a major communication challenge, particularly the need for timely, HIPAA-compliant messaging with hospital staff and community providers.
In 2020 Aegis adopted TigerConnect, standardizing workflows and creating patient-specific group chats that include the full care team and family as appropriate. The platform drastically reduced phone calls, streamlined handoffs and scheduling, and improved family satisfaction; over a seven-day period Aegis sent 138,797 messages to 307 active users, and HCAHPS scores and staff work-life balance both improved as teams stayed aligned in real time.
