Northwell Health, a nonprofit integrated healthcare network in New York, faced a challenge in its preventative patient care initiative. Their existing process for identifying and managing patients with gaps in care relied on inefficient manual methods like Excel exports and macros. They partnered with vendor SoHo Dragon to leverage their SharePoint expertise to digitalize this workflow and provide better access to patient records.

SoHo Dragon extended Northwell's SharePoint implementation, building a solution that automated data synchronization between SharePoint lists and an SQL database. This allowed medical staff to update patient forms from any device, which then triggered queries to identify patients needing proactive outreach. The solution from SoHo Dragon successfully enabled Northwell to offer proactive care for chronically ill patients, syncing information across 500+ doctor locations and allowing the collection of metrics on the efficacy of their new recall procedures.
