Colorado Community Managed Care Network - Customer Case Study

Colorado Community Managed Care Network (CCMCN) is a Colorado nonprofit health information organization governed by the state’s Federally Qualified Health Centers that builds community data infrastructure to address health equity. Facing a need to ingest, normalize, and securely share diverse clinical, social, claims, and public‑health data across healthcare and nonhealthcare partners, CCMCN’s previous environment was slow, operationally burdensome, and not scalable for statewide care coordination.

CCMCN built the Honeycomb Community Data Network on the Snowflake Data Cloud, using Private Data Vaults and Snowflake Secure Data Sharing to let organizations control and govern their data while enabling fast, governed exchanges. The platform’s elastic compute, managed infrastructure, and broad connectivity streamlined pipelines and analytics; today ~150 organizations across 300+ locations use the network, with data on about 3 million people (~40% of Colorado), enabling faster population analytics, coordinated care, and targeted community interventions at scale.
