Medway Council sought to make it easier for people across the borough to access health and wellbeing support as part of a five‑year social prescribing plan. To create a single, population‑level gateway into community services and streamline referrals from NHS agencies, primary care networks and VCSE partners, Medway Council worked with Simply Connect Solutions to deploy the Simply Connect social prescribing and case‑management platform.

Simply Connect Solutions embedded its referral platform within VCS and public health navigation contracts so professionals could refer clients directly, while link workers used the case‑management system to store client data, record case notes and capture health and wellbeing improvements. The solution also added a co‑designed feedback tool to gather service‑user experience, improving coordination across partners and delivering measurable tracking of outcomes and service feedback through the Simply Connect platform.
