Involve Kent - Customer Case Study

Involve Kent delivered the Connect Well West Kent service (established 2018 and funded by the Dept. of Health & Social Care and West Kent CCG) to help people with long‑term health conditions, mental health needs, loneliness or complex social needs. To coordinate social prescribing referrals from five lead GP practices and satellite practices, Involve Kent worked with Simply Connect Solutions, using Simply Connect’s social prescribing service and Link Worker model.

Simply Connect Solutions implemented a referral-to-community workflow where GPs refer patients into the service, Link Workers assess needs and identify suitable community support, and ONS Wellbeing questions are used to baseline and monitor outcomes. The Simply Connect solution streamlined referral routes into local services, provided ongoing Link Worker support, and delivered measurable wellbeing data for Involve Kent and funders.
