Bury VCFA’s Beacon Service was commissioned to deliver a borough‑wide social prescribing service connecting patients with social, emotional and practical needs to local voluntary, community and faith (VCF) organisations. Faced with referrals from 29 GP networks and the need for a single point of access, Bury VCFA required an integrated referral and case‑management solution to coordinate Link Worker assessments, care plans and onward referrals at scale. They partnered with Simply Connect Solutions to provide that social prescribing platform and case management capability.
Simply Connect Solutions implemented its integrated social prescribing system so Beacon Service Link Workers could record assessments, co‑produce personalised care plans, refer into local VCF services, manage caseloads and track outcomes. The platform created a single point of access across 29 GP networks, enabled consistent recording of client data, and allowed Bury VCFA to collect and report on patient health and wellbeing improvements, while supporting VCF organisations to increase capacity and sustainability.
