HealthCare Partners (HCP), a physician-led healthcare organization, faced challenges with operational inefficiency due to its reliance on fragmented, outdated systems and third-party tools for identifying patient care gaps. Their on-premise infrastructure and strict HIPAA compliance requirements made a cloud migration complex. Seamgen was engaged to develop a modern solution to streamline these processes.

Seamgen designed and developed a custom web application on the Microsoft Azure cloud platform, using .NET Core and React. The solution integrated with existing systems like Pulse8 for the MVP phase and introduced a one-way data sync to facilitate the transition from on-premise hosting. This new portal empowered HCP's clinical staff to more efficiently identify care gaps and schedule patient visits. The solution positioned HCP to maximize gap closure rates and begin modernizing its software processes.
