HealthPartners, a consumer‑governed nonprofit health care organization serving 1.25 million members, needed to improve patient outcomes while controlling costs. The challenge was to identify high‑quality, cost‑efficient providers, pinpoint which patients would benefit from case management, and connect clinical and lifestyle data to reveal the factors that drive better health and lower spending.

Using SAS Business Analytics to integrate claims, clinical, survey and lifestyle data, HealthPartners built provider ratings and a tiered benefit plan, targeted disease and case management to the right members, and supported research and EHR decision‑support efforts. The work helped members achieve better control of chronic conditions, saved the organization $137 million annually, produced a 99% success rate among identified members, and generated published research and ongoing clinical improvements.
