Alignment Healthcare needed a reliable, easy-to-use remote monitoring solution for its highest‑risk Medicare patients that would capture daily biometrics, enable secure video calls, feed data into a 24/7 Command Center and integrate with its EHR. To meet those needs AHC deployed a Vivify Health remote care platform on Samsung Galaxy tablets, giving elderly patients a simple tablet‑based kit with Bluetooth medical devices and tailored care plans.

Using Vivify on Samsung tablets, patients transmit weight, blood pressure and oximetry data and answer daily symptom questions so AHC clinicians can spot trends and initiate video callbacks or in‑person care. The Samsung/Vivify deployment now supports more than 120 patients with an 86% compliance rate, reduces hospital admissions and costs, improves patient engagement and education, and even enabled clinicians to detect a life‑threatening GI bleed and arrange immediate care that likely averted serious harm.
