Ascension at Home Wisconsin, part of the larger Ascension health system, needed a scalable remote-care solution to handle growing telemonitoring demand, a workforce shortage, and gaps in patient communication that led to avoidable readmissions. To address this, Ascension deployed a Vivify Health remote care management program that includes Samsung Galaxy tablets with Samsung Knox security and Bluetooth biometric devices, plus clinical monitoring from Evolution Health.

Using Samsung tablets and the Vivify platform, high‑risk patients transmit daily vitals and questionnaires to Evolution Health’s Medical Command Center; Evolution resolves 81% of alerts and escalates 19% for Ascension follow‑up. The program cut readmission rates to 8% (about one‑third of the national average), generated 7,200 alerts from 113 telemonitoring patients in one month, and enabled Ascension to add 300 customers in a year while serving more patients with fewer dedicated resources.
