Health Leads partners with hospitals to address patients’ basic needs—like food, housing, and heat—by “prescribing” resources and mobilizing nearly 1,000 college student advocates across 20 health centers. Growing demand and reliance on spreadsheets and a disconnected database made it hard to enter, share, and track client and community resource data, limiting advocates’ ability to manage cases and measure impact.

Health Leads implemented Salesforce (Sales Cloud, Communities, Platform, Chatter) to centralize client records, resource directories, advocate performance, and donor/hospital relationships in a HIPAA‑compliant system accessible from iPads. The solution streamlined intake and case management, enabled secure advocate collaboration, improved impact measurement, and supports the organization’s work with more than 14,500 families while advancing a new model of whole‑person healthcare.
