Partners Continuing Care - Customer Case Study

Partners Continuing Care (PCC), a large healthcare system, faced a challenge with inconsistent safety event reporting across its many rehab and home care facilities. With some areas using paper forms and others relying on homegrown electronic systems, it was difficult to identify trends and improve patient safety. To support its strong safety culture, PCC turned to RLDatix and implemented its RL6:Risk and RL6:Feedback products to create a unified, system-wide reporting process.

By implementing and customizing RLDatix's RL6 platform, PCC achieved a significant increase in incident reporting and a faster resolution of complaints. The solution allowed for tailored forms, such as one for falls that captured specific harm data, which helped drive targeted improvements that reduced injuries despite an increase in reported events. RLDatix provided the tools for PCC to establish a positive feedback loop, recognize staff contributions, and use data to make informed decisions on everything from care transitions to contract renewals for clinical services.
