Partners Continuing Care (PCC), a large healthcare system, faced a challenge with inconsistent safety event reporting across its many rehab and home care facilities. With some areas using paper forms and others relying on homegrown electronic systems, it was difficult to identify trends and improve patient safety. The solution allowed for tailored forms, such as one for falls that captured specific harm data, which helped drive targeted improvements that reduced injuries despite an increase in reported events.
