A River Logic Case Study
Puget Sound Regional Fire Authority launched FD CARES to deliver in‑home clinical services and care coordination for non‑emergent 911 callers and wanted to measure the program’s value against the IHI triple aim (better care, healthier populations, lower per‑capita cost). To evaluate and prescribe improvements, Puget Sound Regional Fire Authority engaged CGI, using CGI’s Healthcare Enterprise Optimization solution together with River Logic’s Enterprise Optimizer® platform to apply prescriptive analytics to FD CARES data.
CGI built a constraint‑based, episode‑level optimization model linking 2015 911, transport, admission and billing data across FD CARES partners, ran “what if” scenarios, and prescribed operational changes. The 18‑month pilot showed better patient experience and continuity, with FD CARES stabilizing 53% of patients at home (vs. 22% baseline) and transporting only 26% to hospital (vs. 67% baseline); CGI’s analysis estimated up to $600K annual savings to PSF, up to $1M to the health system, and reductions of up to ~200 ED visits annually.
