CommunityHealth, a major free clinic in Chicago providing care to uninsured patients, faced a significant challenge in efficiently connecting their patients to external community resources like food banks and dialysis clinics. Their previous manual process of finding resources through word-of-mouth or online searches was time-consuming and often resulted in referrals to unsuitable or unavailable services. They partnered with Redox and the NowPow platform to address this issue and integrate it with their athenahealth EHR.

Using Redox's integration platform and PatientAdmin data model, NowPow was seamlessly integrated with minimal technical lift for CommunityHealth's IT team. This solution provided clinicians with direct access to NowPow's validated and continuously updated resource database, enabling them to create personalized referrals for patients. The integration, facilitated by Redox, resulted in measurable impact: 153 patients benefited from 433 interactions, and 210 personalized resource lists were created, streamlining workflows and ensuring patients received the appropriate care.
