Case Study: CenterPointe achieves whole-person care and expanded access with Qualifacts Credible EHR

Supporting Whole Person Care at Nebraska CCBHC, CenterPointe Inc., with One EHR

CenterPointe Inc., a Certified Community Behavioral Health Clinic in Lincoln, Nebraska, expanded beyond behavioral health to add primary care services for high-need communities. As it grew its CCBHC program, CenterPointe needed to rework workflows, data collection, reporting, and forms in its Credible EHR from QualiFacts to support integrated whole-person care.

With QualiFacts and its Credible EHR, CenterPointe built new primary care workflows and improved coordination between behavioral health and medical staff under one roof. The results included faster access to care, with an average of 12 days to see a PCP, a median of 4.5 days, and 71.8% seen within a week. The share of individuals with an established PCP increased from 27.2% to 44.8%, helping CenterPointe serve 1,100 more people with primary care and driving 34% more individuals seen in FY22-23 and 42% more brand-new individuals than before becoming a CCBHC.
