Oaks Integrated Care - Customer Case Study

Oaks Integrated Care is a private, nonprofit behavioral health provider in Mount Holly, NJ, offering 230 programs to about 34,000 people. Selected for the two‑year federally funded CCBHC demonstration, Oaks faced the challenge of scaling coordinated mental health and substance use services for people with complex needs while meeting strict clinical documentation, reporting, interoperability and claims requirements to measure performance and outcomes.

Using CCBHC funding and Qualifacts’ CareLogic EHR, Oaks expanded services (ambulatory detox, MAT, 24‑hour triage, transportation and care partnerships) and implemented EHR tools for documentation, data collection and reporting. After one year the organization is serving more people, documenting more than 30 client success stories, improving access for previously underserved individuals, and demonstrating measurable coordinated‑care outcomes.
