Family Services of Westchester, a nonprofit provider of comprehensive behavioral health and social services in Westchester County, NY, needed a better way to understand the care its clients received outside its own facilities. Working with QualiFacts and its CareLogic EHR, the organization aimed to improve care coordination and gain a more complete view of client medical history across the broader healthcare system.

QualiFacts implemented a direct integration between CareLogic and HealthlinkNY, a Health Information Exchange, giving clinicians access to additional patient information from community providers and other connected networks. The result was better-informed clinical decision-making and stronger collaboration across care settings, with the added benefit of HealthlinkNY’s participation incentives, including a $13,000 grant and no membership fees.
