King’s Daughters Medical Center is working with Qualcomm Life, Inc. to reduce readmissions and improve in‑home management for heart failure, diabetes, COPD and hypertension. Serving a largely rural population of about 750,000 across six counties, KDMC faced high 30‑day readmission rates, barriers to access, and poor compliance with discharge protocols—creating a pressing need for remote, easy‑to‑use monitoring.

Qualcomm Life, Inc. implemented its HealthyCircles Care Coordination Platform and the 2net Connectivity Platform plus connected personal health devices (pulse oximeter, smart scale, blood pressure meter) as a Heart Failure Kit, streaming vitals into clinicians’ dashboards and the EMR for exception‑based alerts and targeted interventions. The initiative is expected to reduce heart‑failure readmissions by 25%; early pilots showed one HF patient with nine admissions had no further admissions after enrollment and a diabetic patient’s A1c dropped from 11.2% toward an anticipated ~6.8%, while improving patient satisfaction and enabling cost savings through “admit to home” care.
