Contra Costa Health Services, the integrated public health system for roughly 1.1 million East Bay residents, struggled to turn growing volumes of EHR and community data into actionable insight. In 2016 Contra Costa partnered with Qlik to build intuitive, human-centered dashboards and embed analysts with operational teams. The result: 25+ focused dashboards (including a Whole Person Care case‑manager tool and public COVID-19 dashboards) that deliver real-time, actionable data—helping target the top at‑risk patients, meet quality targets, preserve funding, expose disparities, and provide public transparency (2.5 million dashboard views in four months).
