Oberlin Road Pediatrics struggled with fragmented communication when referring patients to the Emergency Department, which risked lost clinical information, longer ED wait times, lower patient satisfaction, and higher costs. To address this, WakeMed adopted Proficient Health’s PH Connect mobile app to give primary care physicians a secure, direct way to transmit patient health information to EDs and urgent care locations.
Proficient Health’s PH Connect was integrated into WakeMed’s workflow and routed through the OneCall center so nurses could post incoming messages to the patient’s EHR and ED tracking board. The change led to measurable improvements: more than 3,000 patients arrived with clinical information already in the EHR, average response time to PCP messages dropped to under three minutes, and teams reported faster time to treatment, fewer unnecessary diagnostics, better resource use, higher physician engagement, and improved patient satisfaction.
