The Health & Fitness Center of Washtenaw Community College - Customer Case Study

The Health & Fitness Center of Washtenaw Community College faced a common post‑rehab gap: insurance‑limited therapy left many patients medically cleared but not yet able to safely resume daily activity or independent exercise. Power Wellness partnered with the center to address this transitional challenge by supporting the launch of a dedicated Transitional Care program to bridge the gap between clinical rehab and community‑based fitness.

Power Wellness helped implement six targeted post‑therapeutic programs (Weight Management, Orthopedic Fitness, Cancer Fitness, Cardiac Fitness, Transitional Care, and Diabetes Fitness) emphasizing education, individualized attention, and coordination with referring clinicians. The initiative enrolled 26 participants, produced measurable functional gains (for example, one participant, Leslie, regained strength, endurance and balance and no longer needs an aide), and converted 64% of Transitional Care participants into long‑term center members.
