Case Study: Northwell Health improves community health insights with PolicyMap

Northwell Health, New York’s largest healthcare provider, wanted a better way to understand and address the social determinants of health affecting the communities it serves. Using PolicyMap’s geospatial analysis and mapping tools, along with curated demographic, quality-of-life, and health datasets, the Community Health team began building a more complete view of neighborhood-level risk.

With PolicyMap, Northwell Health is developing a Social Vulnerability Index and Patient Risk Score to combine community data with anonymized patient-level information. The solution will help care providers quickly identify patient risks, rank issues by priority, and spot community hotspots to guide referrals and community benefit spending. While the case study does not cite specific numerical results yet, PolicyMap is enabling Northwell to create a more end-to-end care referral platform and data-driven feedback loop for improved community health.
