Clinically Home, a home health organization delivering acute care in patients’ homes, needed a digital platform to improve care coordination, reduce errors, and boost provider productivity. They engaged PointClear Solutions to provide solution strategy, UX/design and development support to create an easy-to-access, web-based specialty EMR and supporting services.

PointClear Solutions used an iterative, multidisciplinary approach—task analysis, observational research, interaction and visual design, and development—to deliver a web EMR with remote documentation, order entry, e‑prescribing and document management that streamlined physician workflows and team-based care. The system increased provider efficiency and satisfaction, reduced care-management and documentation costs, closed gaps in care, and improved patient outcomes.
