Care Coordination App Gives Providers Power to Monitor Patients Remotely

Abbott needed a mobile monitoring solution to improve communication between patients and care coordination teams and to support medication monitoring and adherence. Abbott engaged PointClear Solutions to design the myMerlin mobile app, a clinician- and patient-friendly application that integrates and syncs with Abbott’s CardioMEMS HF System, the FDA‑approved heart failure monitor.

PointClear Solutions ran a week-long onsite strategy and planning session and provided ongoing UX design and mobile architecture support to build myMerlin. The app enables care teams to monitor devices remotely while patients remain at home, provides medication schedules and alerts, and tracks adherence via history/calendar screens—complementing the CardioMEMS HF System’s proven ability to significantly reduce heart failure–related hospital admissions and improve quality of life.
