Children’s Health Alliance faced the new frontier of pediatric population health management: no standardized pediatric PHM models, the need to develop pediatric-specific quality measures and workflows, and the challenge of coordinating care across more than 100 independent pediatricians in 20 practices using eight different EMRs. To meet these needs the organization partnered with Philips Wellcentive and adopted its integrated population health management solution (implemented as the Care Management Analytics and Reporting Tool, or CMART).
Philips Wellcentive aggregated clinical and claims data across the community, enabled multiple condition registries and risk stratification for 80,000+ patients, and delivered actionable alerts and shared care plans into practice workflows. The Philips Wellcentive solution helped CHA transition its asthma registry, integrate ED alerts and payer data, and drive measurable improvements: ED visit rates for the asthma population were 20–40% below benchmarks, the number of patients receiving evidence‑based asthma protocols rose 80–800%, home action plans increased ~360%, and annual asthma well visits grew ~230%.
