A Philips Wellcentive Case Study
Blanchard Valley Health System partnered with Philips Wellcentive to launch an employer-based patient-centered medical home aimed at moving from retrospective quality measurement to prospective, value-based care. The challenge was to reduce variation in diabetes and hypertension care, improve preventive screening and immunizations, and lower avoidable ER visits and inpatient admissions across 17 participating practices and more than 4,000 enrollees in the employer health plan.
Philips Wellcentive’s end-to-end population health management solution provided an actionable patient registry, gap-in-care alerts, reporting, and care-management templates that empowered embedded nurse care navigators to target high-risk patients. Using Philips Wellcentive, Blanchard reduced A1c averages in complex diabetic cases from 8.5 to 7.9 (with up to 18% improvement in the worst-controlled subgroup), increased colonoscopy rates from ~47% (2009) to 67% (2013), raised mammogram compliance to 77.1%, boosted Tdap coverage to ~45%, cut inpatient admissions ~22.8% and ER treat-and-release visits 8.1%, achieved a $2.44 return for every dollar invested, and lowered PMPM costs for continuously enrolled members.
