Commissioning better care for over 45,000 people with complex needs

NHS Nene Clinical Commissioning Group, working with two other commissioning bodies in Northamptonshire, needed a better way to coordinate health and social care for more than 45,000 people with complex needs, including older people leaving hospital and people with learning disabilities. PA Consulting was brought in to help the local system improve collaboration, define desired outcomes, and plan more integrated community-based care.

PA Consulting supported the organisations with an Integration Transformation Programme by developing outcome-based service definitions, a provider guide, and a rewards model for care providers. PA Consulting also built scenario models to test the impact of shifting care from community hospitals into people’s homes, helping commissioners decide the best way forward and improve shared budgets, market development, and contracting. The work is now enabling better care for around 45,000 people while helping stretch constrained health and care budgets further.
