Integrated Assesment Record is a province-wide initiative to ensure assessment information follows clients as they move between health service providers. Facing fragmented community-care data silos, strict privacy/security requirements, and the need for cross‑organization interoperability, the program selected Orion Health to provide its Rhapsody Integration Engine, Clinical Portal and Clinical Data Repository to enable secure, standardized sharing of assessment data.

Orion Health provisioned, configured, tested and deployed the integration engine and Clinical Portal, collaborated on an Implementation Planning Study, and adapted interfaces to reduce rollout time and cost. The solution delivered the first Canadian electronic sharing of community care assessments, is being expanded from pilot sites to over 2,000 health service provider organizations and more than 8,000 clinicians, and improved secure access, coordinated care planning and on‑time, on‑budget delivery.
