Innovation Care Partners, a physician-led clinical integration network in Arizona, faced the challenge of coordinating care across nearly 2,000 independent physicians using dozens of different EMR systems. They needed an open, clinician-friendly platform to facilitate real-time data exchange, identify at-risk patients, and meet the demands of value-based care contracts. Orion Health provided an open-architecture platform that aggregated patient data from disparate EMRs, enabling effective care coordination. This solution allowed Innovation Care Partners to build custom tools for alerts and reporting. The results were significant, including over $82 million in savings in the Medicare Shared Savings Program over five years, shared savings payments of $37 million, and lowered utilization costs for multiple payer types, while achieving some of the lowest readmission rates in Arizona.
