Innovation Care Partners leverage a population health management platform to coordinate care in a value-based environment

Innovation Care Partners (ICP) is a physician‑led clinical integration network in Scottsdale, Arizona, serving over 150,000 patients with nearly 2,000 participating physicians across ten payers. ICP needed an open‑standard, clinician‑friendly health information exchange and population health platform that would integrate dozens of different EMRs, identify high‑risk patients, close gaps in care, simplify quality reporting, and meet ACO/MSSP contract requirements. To meet those needs ICP evaluated solutions including Orion Health’s products, ultimately implementing Orion Health MSSP Management and Orion Health Health Information Exchange.

ICP partnered with Orion Health to deploy an open, standards‑based HIE and population health platform with a web clinical portal, timeline views, notification hub (integrated with TigerConnect), open APIs, real‑time alerting and custom census reporting. The Orion Health solution helped ICP gain Medicare ACO status in 2014, realize nearly $3.75 million in first‑year savings, and achieve cumulative Medicare Shared Savings Program savings of over $82 million in five years (ICP’s share $37 million on >$1 billion benchmark spend). ICP also lowered costs 14.5% for a Medicare Advantage payer, 5% for a commercial payer, cut costs 6.5% in one year for a self‑funded plan (saving $4.6M with $2.1M shared savings), and recorded some of the lowest readmission rates in Arizona—outcomes driven by Orion Health’s platform-enabled care coordination and analytics.
