A Oracle Case Study
Margaret Mary Health, a health system in Batesville, Indiana, set out to improve outcomes for high-risk diabetes patients by reducing the share of primary care patients with poor A1C control. Working with Oracle Health, the team used registries, scorecards, population health analytics, and an enterprise data warehouse to identify gaps in care, compare provider performance, and target patients who were overdue for testing or had A1C levels above 9%.
Oracle Health helped Margaret Mary Health launch proactive outreach, patient-specific treatment plans, community diabetes clinics, staff education on capturing external records, and expanded point-of-care testing in primary care offices. The effort reduced the percentage of diabetes patients with A1C over 9% or no completed test from 36% at baseline to 23.9% by December 2022, surpassing the goal of 27.5%, and improved further to 24.5% by August 2023.
